Information on Chronic Disease Management

Patients with conditions such as:

• Asthma & COPD (Chronic Obstructive Pulmonary Disease)
• Diabetes
• Hypertension
• Cardiovascular disease
• CKD (Chronic Kidney Disease)

all need regular checks to monitor their health and wellbeing.

Regular review is likely to involve an assessment of wellbeing as well as certain blood and urine tests, a blood pressure check or lung function tests.

Chronic Disease monitoring is largely carried out by the Practice Nurses. Bethan Bell, the Clinical Practice Pharmacist, sees patients with CKD, epilepsy and poorly controlled hypertension.
